THIS NOTICE DESCRIBES HOW MEDICAL AND HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

While Coastal Life Healthcare provides non-medical care, we are committed to maintaining the privacy and confidentiality of our clients’ Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

 

1. Uses and Disclosures of Health Information

We may use and disclose your health information for purposes of providing care, coordinating services, and operations. For example, we may share necessary information with family members or other caregivers involved in your care plan, provided we have your consent.

2. Your Rights Regarding Your Health Information

You have the right to:

  • Request restrictions on certain uses and disclosures of your information.

  • Receive confidential communications of your health information.

  • Inspect and copy your health information.

  • Request an amendment to your health information.

  • Receive an accounting of disclosures of your health information.

3. Our Duties

We are required by law to maintain the privacy of your protected health information and to provide you with notice of our legal duties and privacy practices with respect to PHI.

Complaints: If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services.
